Have no form of health insurance or healthcare coverage (Medicaid/Chip, Private Insurance, etc.)

Document to submit:
Present any valid Photo ID which includes the date of birth (Passport, U.S. Permanent Resident Card, Driving License, Student ID, Nepali Passport, etc.)
